IW Prescription Direct (P.O.D)

The Prescription Direct Care Home Service processes medication orders, takes medication queries and issues prescriptions for GPs to sign. The aims/benefits of the service are: 

  • To free up practice time by taking over all calls, emails and orders regarding medication for care homes
  • Dedicated email monitored Mon-Fri 9am-4pm. Clear, quick replies to queries and follow up communications once query has been resolved. Care homes, pharmacies and surgeries are always kept informed
  • Suggest opportunistic switches according to the IOW Prescribing Formulary (often same medicine but cheaper brands or generic)
  • Suggest de-prescribing of medications that are no longer required or recommended by NHS England
  • Complete Polypharmacy medicine reviews for care home residents and send recommendations to the GP. Medication reviews stop over medication of patients, which reduces tablet burden and saves time on drug rounds
  • Add on repeat medication templates for new residents. POD will collate evidence of current medication and email these to the practice to be added to the patient records e.g. discharge summaries, confirmation emails of medications from clinics, copy of MAR from the care home
  • Offer GPs advice eg. formulation switches when a patient cannot swallow tablets or alternatives when there is a stock issue
  • Identify any errors/interactions. POD identifies errors/interactions and reports them to the prescriber, which helps identify trends and reduce future errors
