WISDOM project recognised by NHS England

The West Hampshire Improving Shared Diabetes Outcome Measures (WISDOM) project has been praised by NHS England for improving three key indicators blood glucose control, blood pressure and cholesterol - through improved patient medication and healthier lifestyle choices.
West Hampshire Community Diabetes Team working closely with GPs and practice nurses to help people with diabetes in west Hampshire to access refresher education and improvements to their treatment to help them better control diabetes and improve their quality of life.
The WISDOM project, which is delivered locally by Southern Health NHS Foundation Trust, is commissioned by NHS England. Professor Partha Kar, National Diabetes Specialty Advisor, NHS England, said, "It is heartening to see areas benefitting from the NHS Transformation funds. For the Hampshire region, allocation of this into projects such as WISDOM has shown an improvement in care processes as well as achievement of relevant targets in Type 1 and Type 2 diabetes – which is hugely encouraging".
